Grievance Procedure

While Nurtured Roots strives to provide competent, well-rounded care to our clients, we encourage client feedback and self-advocacy. Should there be concerns about care, believed breach of confidentiality, or abuse, a grievance form can be completed. Please read carefully to understand steps that can be taken to address a grievance with Nurtured Roots Counseling.

Level 1:

If there is a discrepancy between the client and their therapist, concerns need to be addressed directly with the therapist. Both parties can discuss their concerns and come to a reasonable resolution that meets both parties' needs. If the therapist is a student intern, the issue will be addressed with the intern’s Clinical Supervisor.

Level 2:

Clients of Nurtured Roots Counseling reserve the right to talk to the Director of Nurtured Roots Counseling should they feel the conversation with the therapist (Level 1) has not adequately addressed the situation. The Director will attempt to remedy the concerns between the therapist and client.

Level 3:

Clients should place all grievances and steps that have been taken by themselves, and Nurtured Roots Counseling, in writing using the “Grievance and Complaint Summary Form”. This process must be completed within one month of the meeting with the Director. The referenced form can be obtained from the Client Rights Specialist/Director. The Grievance will be reviewed by the Director and meet with the parties involved to discuss resolution. Following the completion of the meeting, a letter will be mailed to involved parties identifying Nurtured Roots position and conclusion of the transpired events.
